Help with MIC

I Have found a MIC for Lt. Frederick Walduck MC RFA. There is a date written on it,

19.4.21 but I cannot decipher the other text in this line. What does it mean?

Was he discharged or died? Looks like he survived the war in any caes.

the Date is the issue date of his Award{s},the Code preceding & after the date are the Medal Roll Page references the MiC alludes to;

It appears there was an amendement to his entry for the star as there was an authority given for it (see line next to " reason"). I have a feeling that Lt was put on the star instead of 2nd Lt as his seniority date for his highest rank was 1 June 1916.
