This soldier was admitted to the 19th General Hospital in Alexandria with a gunshot wound to the head. 

 

I'm having a hard time deciphering the 'Remarks by Operator'. I can make out '?? enlarged', 'fracture of outer ???', and 'No bone removed', but that's about it. Any ideas?

Apart from one missing word(s), I think it reads:

 

   'Wound enlarged. There was small ??? fracture of outer table. Wound left open. No bone removed'

 

Must have been a superficial wound. Seems like he was a very lucky man.
