Royal Pension Warrant - a rare item these days?

I have being researching the grandfather of a friend and she lent me some items to assist me in my research. One of the items was his actual pay book and folded inside was an application for disablement pension. This is the first time I have actually seen such a document and I thought that I would post images of it along with a photo of the man himself.

Pension application forms all round are unusual to see simply because the records are typically destroyed - the few that you do see are in the PIN26 files.

Article 9 was for a post-discharge pension application.
