PENSION CARD - Interpret card

Please can someone interpret this card [a style I haven't seen before]

OK here is a partial start on the forms:

Army Form W 3972A = Seems related to W 3972A Notification to Officer i/c Records Soldier's Discharge Under KR Para 392 (xvi)

Army Form 179C = Seems possibly related to AF B 179, 179A & 179B which relate to Medical Reports

 

And a stab at:

DD 31-12-18 = Possibly DD = Died? Or Disabled?

 

7/MD/1916 = later a regional case number = Region 7 / M[ilitary] / D [? - would perhaps have expected C for surname Clarke first letter but as D wonder if this is Dependants??] / Case ref. no.  1916

BH2B5665 = obviously a case number but perhaps an earlier one??

Form 2A/Registary Chelsea (11048) = 11048 seems to be a Chelsea case number - possibly earlier still??

???

Further enlightenment, especially on sequencing of case references, is welcomed.
