Document interpretation ?

It is 3137 Isaac W J Clack who is the subject of this and we are told that the document was returned to the next of kin of said I W J Clack.  The concentration sheet is clearly in error, the number and initials are corrected on the main record.

 

 I would agree that this is a file note and perhaps as you say, a draft for the parents.  The record office would most certainly not use map references in something like that, hence the (not wholly accurate) description of his resting place.
