Help with medal index roll for James Shrosbree needed please

I have attached a medal index card for my great uncle, James Shrosbree. I could use a little help deciphering it please. I think because he has two numbers he must have served with both 6th London reg and MGC.

He forfeited one medal because he was recorded as deserted after returning from leave late, having got on the wrong train while a bit worse for wear.

Any help with the rest of it would be great though.

Went to France 17 3 1915 (France/Flanders denoted by the number 1 in Theatre of war). First enlisted in the 6th London Regt then transferred to the MGC. His forfeited medal was restored to him as per comment "do Rest" (do as in short for ditto).

His 15 star will be recorded in the medal roll here - http://discovery.nationalarchives.gov.uk/SearchUI/Details?uri=C4438050 ,but what has happened to his pair? They are not listed in the same book, so he should have another MIC or a reference to them on that card?? Found it, has the pair and service details as with the above card under Shosbree . No other comments. Roll MGC101B49 page 4184 =
