Daniel Sheridan's medal index card help

Hi again,

I now need some assistance in decifering some addition codes and writing on my gr. gr. uncles medal index. I know his service number and stationments already and that he was entilled to pip, squeek and wilfred. what im wondering is about the other writings and lableings which i could not understand using 1914-1918 aids. i have attached two pics regarding the given index card.
