Can't find MIC or MID card! Help PLEASE

I'm trying to locate the medal index card/MID card of the following officer to no avail -

Richard Sapcote RA - MID LG 7/4/18

& the MID ( June '16)card for Brig Gen John Joseph MacMahon RA, got his MIC ok

I can find neither man in the LG for their MID's but confirmed elsewhere

I have tried various mis spellings

Officers had to apply for their medals, and not all did so.

So looks like what was stated above, but I thought they would still have a card done, but obviously not.

Thanks to all those above, especially for the LG's!

Re Sapcote's MIC, he served into the '20's so I suspect his MIC has a WRONG spelling, his Hickie Parchment has 'Sapcite'! but I've tried that.
