it probably means that he was promoted in the field i.e. in the trenches and did not attend a promotion course or a specialist course to achieve promotion. This was not uncommon given the need for NCO's and the number of casualties.

He could also have been promoted for Gallantry, far better for the family than a mention in despatches would mean an increase in pay and status.
