Lt Frank Luke aka "The Balloon Buster" received both the Medal of Honer and the DSC.

In regard to a third decoration:

Was it the French or Italian Croix De Guerre?

According to A E Durkota's Medal of Honor - Aviators of World War I, Lt Frank Luke was awarded the Medal of Honor, the Distinguished Service Cross with Oak Leaf Cluster, the Italian Croice de Guerra, the Aero Club Medal for Bravery and, as the first graduate of Rockwell Field to be credited with an aerial victory, Rockwell's Margarita Fisher Gold Medal.
