For the purposes of biography, does anybody know what the official citation might have been for crewman on ship torpedoed ,lost at sea. There was one in the house once,gone now. . Also when a letter was sent home to inform relatives of loss, what might it have read?

Presumably you mean the Official Commemorative Scroll,sent to the Next of Kin that accompanied the Commemorative Plaque.

The Army form that notified of a death in Action was a very straight forward & blunt document,printed with just the nominal & regiment details written in,presumably the Naval form was similar
