I'm not really an "active" collector, but have an interest in military medical paperwork of all periods. I have some original Army Forms (mostly non-medical) as follows:- WW1 era: B.2510 Advice of Army Forms D.418B and O.1838A despatched... (no date, but definitely pre-1921) D.418 Statement for Separation Allowance... (2/09) O.1634 Gratuity on Discharge or Transfer to the Army Reserve (10/14) O.1832 National Insurance Act 1911 - Declaration to be made by a soldier on completion of his first term of engagement (6/14) O.1836 National Insurance Act 1911 - Return to be rendered with the first contribution card... (5/15) Later: B.72 - Form of commitment to detention barrack... (1/40) B.115 - Record of the declaration of a court of inquiry... (10/39) O.1609 - Payments to local clergyment... (4/40) O.1667 - Civilian medical practitioner's bill (5/40) AB 27A - A & D book (all medical units in the field, rev. 1944) Also some photocopies of WW1 hospital cards from the PRO. I don't have regular access to a scanner, but will see if I can get any of this scanned in sometime. If anyone has examples of medical, dental or veterinary forms (other than the B.178) I would be very interested in seeing them -- also pre- and post-WW1 material. Sorry, why do you need to do that? Just place your images in one of the free spaces I have suggested and use the IMG button to show them in your posts. They will appear exactly as they do now.
