And the Army Form B.213 Field Return also had an associated perforated sheet to record the numbers needed to get back up to establishment strength ..

Trying to find a copy or definition of A.F.W 3695 (see left hand column).  Many thanks in advance. I have check all 11 pages of the thread.

Army Form C.2120 - Telephone Message.  The telephone equivalent of the familiar C.2121 Message & Signals form.

Found in 33rd Division HQ war diary.

 

Also this improvised version, which does not have an AF number, but was clearly used for the same purpose.

Army Form W.3459 - Visits of Relatives to Sick & Wounded Soldiers in France

Thought I had solved the mystery, but alas no..

 

Got excited when I found this... Army Form W.3659 (not 3695 perhaps a typo on the document?) Is the form for an Absentee Rejoining his unit.  See below.

Another unusual form uncovered in a service record, this particular chap, originally in the MMGS in 20th (Light) Division, but compulsorily transferred to the Manchesters, had been a boilermaker (and then a policeman) in civvy street.  After a Blighty wound in the thigh, and some time on light duties at Home, he was discharged with a SWB and this job in a Middlesbrough shipbuilders ...

 

Army Form W.3916 - Release of Soldier for Shipbuilding in three parts A, B and C, but Part C (Voucher for presentation by the soldier at the Discharge Centre) from this set is missing

Hi,

 

Sorry if this has already been mentioned but I’ve just become aware that Australian Archives have some complete Court Martial files available online.

 

Each contain a variety of Army Forms - this one has a couple of Magistrates Guidance forms for Absentees/Deserters the second of which is completed with a recommendation for a reward for the Police Officer who arrested the soldier.

There is also a completed “handover” schedule used by the Sergeant of the Guardroom - from which the soldier escaped - that I’ve never seen before.

Hi, I am seeking a copy of SS 400, Instructions to ASC MT drivers on the Western Front. This document was to be kept in all Mechanical Transport vehicles and was issued from 1916. I have searched all the lists I can find to no avail. Does anyone have a copy please ?

I've posted a couple of examples of form SD 622 which was the confidential report on cadets passing out of (or failing) officer cadet battalions (OCBs).
