Help reading a medical diagnosis

Could anyone please help me to decipher the writing below taken from a pension record:

Give Diagnosis and particulars of:
(a) Each disability claimed or discovered.
1. GSW R. thigh
2. Gastric distension?

(b) The present condition thereof:
1. Scar on back & front of thigh ?oly? healed. Says he has pain after walking ? distance.

2. Has to be careful about diet. ? de?

I expect you have got GSW = gun shot wound.

Looks to me as if you are right about gastric distension (distended stomach area, probably digestive problems).

It really is odd handwriting. The word before "healed" could be mostly / snugly?

"some" distance?

Agree "Has to be careful about diet." And I think the last two words are "Tongue clean" - tongue condition was quite often used to diagnose gastric problems.

Tongue Clean.  Of course! I would never have got that myself.

Not sure about the other two words. I thought perhaps it could be "nicely" healed ...
