Help reading application for headstone

Henry Kolomoku Umuiwi/Umuivi was an ethnic Hawaiian who served with the US military and died in France in 1918. His body was repatriated to the US in 1921 and he was the first military burial at Homelani Cemetery in Hilo, Hawaii.

In 1941, an application was made to the US government to cover the costs of a plaque for his grave. Can anyone read the name of the applicant that is handwritten as well as the bit underneath that is crossed out? Thanks!
