This is a page from the Fargo Hospital autograph book that I recently acquired.

I'm puzzled about what "1st AM" might stand for; "First Assistant Matron"?? Or Burt might be a soldier-patient.

 

(And I'm not sure if the second initial is a "J".)

Burt	AM1	EF				14TDS Lake Down/UK	03.12.18	inj [E1851 Avro] Seriously inj on leg when propellor started whilst sucking 

 

RFC/RAF Lake Down was three miles south of Fargo Hospital.
