I don't believe I have seen one of these forms before, but I have seen the equivalent that was in use with the Admiralty. Ernest Lucas was a pre-war regular. His attestation states he was born in Battersea in January 1887. On 11 February 1913, he was permitted to extend his service to 12 years with the colours. On 6 June 1916, he was permitted to extend his service to 12 years with the colours. He was later commissioned, and the document below is an eyewitness account of him being killed in action.
