POW form on surviving service file (WO 363)

I came across the following form that I have never seen before.

It is part of the service file for John Isaac Smith, 240296, a TF Private in the West Yorkshire Regiment. I wondered what other forms are out there for POWs.

Here is a form of similar content that is in the surviving service file of 11871 Obed Jenkins, South Wales Borderers, captured at Gheluvelt at Ist Ypres.

The 'Hut No.' indicates to me that this was a form drawn up by the PWs in his camp as their own record. Do you know which camp it was? I can't find his ICRC card.

Sorry, I don't know. Confusingly, the service record's Military History Sheet has him in Germany until 21 November 1918 and returning to England on 22 November 1918, which would suggest to me this form was completed when he returned to the regimental depot. If he was repatriated to Hull, it won't have been so far to travel.

I'm finding him hard to pin down. However, there is a 240296 John Isaac Brown 2nd West Yorks. Have you found an MIC?

Addition 2: He was Brown, not Smith, and I agree with your UK regimental depot (or demobilisation unit) idea. If so, and I don't know much about the regiment, you might look at Imphal Barracks, York (WY Depot), Colliergate in York, Belle Vue Barracks in Bradford or Carlton Barracks in Leeds!

19/12/18 is his arrival date at Ripon Camp on repatriation from Germany. So I assume these forms were prepared in the UK at the clearing camps.

I was looking at a Mr Smith and a Mr Brown, and mixed them up, despite the name being clearly stated in the image in the original post, so I apologise for the confusion.

I was surprised to see that the fields in the second table are the same as in the first. I did wonder if there was an Army Order to specify these fields, but it may simply be that the camp commandant at Ripon wanted to capture these specific fields for the repatriated men that were arriving, if they are both Ripon men.

Not neccesarily so but I suspect there was a standard form for registering returned PoWs.
