I am reading it as : Gunshot wound Head. Nerve left and partial Paralysis of 3rd Cronial of sight. I am probably wrong with some of the underlined words, especially Cronial. I think, the sentence reads GSW Head Paralysis of 3rd Cranial Nerve left and partial [the word 'loss' is missing!] of sight. The 3rd cranial nerve (oculomotor nerve) is responsible for supplying all of the eye muscles apart from Lateral Rectus muscle, which deviates the eyeball straight right or left (6th nerve) and Superior Oblique, (4th Nerve).
