Can anyone please help with a date of Gazette for his MM, a newspaper article at the time of his death stated he was also awarded a Bar to his MM(I know these can be inaccurate) but CWGC only show the original award.

His MM medal card hasn't been "barred" with a diagonal bar across it showing details of a  reward of a Bar to the MM so I would say that is "only" one MM award.
