Help in reading a casualty form

I have just received the discharge/pension claim forms for 8779 Private James Wallace Kay and I am struggling to read certain details. Can anyone help? Many thanks

I’m afraid the quality isnt so good - presume from your mobile phone?

 

It’s too blurred for me to make out much.

So which part in particular do you want help with?

Have to agree with Steve as well, the image you've posted is no help at that resolution. Far better to just give people enough information to find the record themselves.

Hi,

 

Whilst there are other pages, I think that one reads as...

12.10.1914 - admitted 8 General Hospital, Rouen with (?) image.png.81eb829084999b75d4e5788f67a66ab8.png

24.10.1914 - sent to Convalescence Camp

27.10.1914 - sent to (Le) Havre for return to duty

11.12.1914 - rejoined Battalion 'in the field'

22.4.1915 - admitted 14 Field Ambulance as the result of a gun shell, or gun shot that caused a 'graze' to his footimage.png.34f85463eeba07c2dd183d4824bcd898.png

30.4.1915 - rejoined Battalion

28.12.1915 - wounded in action

29.12.1915 - admitted to 15 Field Ambulance with a wound to his thigh

1.1.1916 - admitted to 21 Casualty Clearing Station with a wound to his hip

5.1.1916 - discharged from 8 General Hospital for return to the UK onboard the SS Copenhagen

As he was admitted to 14 FA there should also be a MH106 record for him.

He doesn't show up on MH106 with a search by name & number but he is on the list. MH106/11 page 42 Findmypast.

Listed as 2nd Manchesters, B. Company. Transferred to Divisional Rest Camp same day as per the pension file.

I think unfortunately not. The "Casualty Form - Active Service" appears in some (though not all) of the files that survived a fire in WW2. I didn't see a file for George. What you do have though is his unit (the nature of his wound), and the date that he was admitted to the General Hospital. Given that date, and the nature of the evacuation chain, you could probably make a reasonable guess as to when he actually received the wound, and cross reference to the unit war diary to see if it might point towards something. It is unlikely to mention George specifically by name though.
