ss002d6252 Posted 21 November , 2018 Share Posted 21 November , 2018 (edited) The main effects forms were the W5063 and the W5070 , here's a blank one I mocked up for my website of the W5063. If any one ever comes across a W5070 I would love to get a copy, all the copies seem to have been long since destroyed and I've checked with several different museum and archives to try and obtain one without luck.
