As Lieutenant. I searched the MIC's by surname only but no likely matches, couldn't find a Gazzette mention. Any info at all welcome please. Guy. Lt. 27.7.1918 https://www.thegazette.co.uk/London/issue/30877/supplement/10232 and relinquished his commission 1.9.1921 as Phil has posted in #2 Edit to add:- His M.I.C. is as Harold Gerard Waddington, L/Cpl. 2779, Duke of Lancaster's Own Yeomanry, prior to being commissioned.
