Need help in interpreting medal record for Alwyn Holmes

Attached is my grandfather's record re a medal request. I would be grateful for any additional interpretation aside from the medal and clasp requested. I had been told he was wounded; but, that is about all the information I have thus far. Attempts to trace his movements have been fruitless; so any help there would also be useful. Thanks.

Did you know his service records are at National Archives - http://discovery.nationalarchives.gov.uk/SearchUI/Details?uri=C704423

If you cant get there yourself you can hire a researcher to get them.

That should give you more information on his movements etc.

The card only says he started in the 5th Battalion London Regiment, got commissioned and then was in the 15th Battalion Essex Regiment.
