Chris Henschke Posted 31 October , 2013 Share Posted 31 October , 2013 The A 40 example, and other examples that Croonaert has pointed out, is why I tried it include the date range for forms. As this is a forum for Great War interest, I have excluded forms that are clearly much later vintage, for example, Army Form G 875A Report on Examination of O.M.L. … -Inch Mortar (adapted) (revised Apr 1944). I propose that the known dates of the imprint should be included in your list. Chris Henschke
fv1620 Posted 31 October , 2013 Share Posted 31 October , 2013 I have the definitive list that includes changes up to AO Oct 1916. It is over 200 pages I can't scan the whole thing. But can do request pages. Although can't do much of anything at moment my phone line is down since Monday & nobody seems interested in fixing it. Just struggling with a slow weak 3G. Chris Henschke Posted 31 October , 2013 Share Posted 31 October , 2013 There are 23 sub-titles for Army Form L 1398. This series lists the quantity and types of Army Books and Forms carried in the field by various units. I have only ever found one, Army Form L 1398-23, for Army Service Corps units on Line of Communication. I 1202 Diet and Extra Sheet for Patients in Hospital and Extra Sheet for Dining Halls, and Kitchen Sundries (12/14) I 1209 Indent for Medical Stores (in pads of 100) (6/16) I 1220 Medical Statistical Index Card (Statistics of the War) I 1238 Syphilis case sheet (5/15) K 1305 Road report (G&P, c.1889) L 1398 (Series 1 – 23) -23 Tables of Regulations, Army Books and Forms, and Stationery (Field) (ASC units on L of C) M 1430 Estimates of pay (G&P, c.1889) N 1474 Pay-Sergeant’s Cash Account sheets (G&P, c.1889) N 1487 Requisition for Cash (AFL 1398-23) N 1504 Pay and Mess Book (G&P, c.1889) N 1505 Pay List (G&P, c.1889) N 1531A Sums received and expended by Sub-Accountants monthly statement of cash received and issued (duplicate of payment in the Field kept on this document by O.Cs Squadrons, Batteries, Companies) (AFL 1398-23) (Army Accounts, Army Review Vol. VI No. 1 Jan 1914) (FSR Pt II) N 1535 Pay Sheet of Civilians employed by Departments (AFL 1398-23)
IPT Posted 1 November , 2013 Author Share Posted 1 November , 2013 Thanks, Chris. I will look into adding the dates. Was L 3198 in the form of a book, containing all 23 pieces? Either way in 1911, it appears to have consisted of 76 pages - http://books.google.co.uk/books?id=Qhi4tgAACAAJ&dq=%22Tables+of+Regulations,+Army+Books+and+Forms%22&hl=en&sa=X&ei=1wNzUrmuMeaJ0AWOjoHwAQ&ved=0CDEQ6AEwAA This 1914 version gives a breakdown of some of the units - http://books.google.co.uk/books/about/Tables_of_Regulations_Army_Books_and_For.html?id=ykgskgAACAAJ&redir_esc=y AF L 1398-11 seems to have been for an infantry battalion, in 1938 at least - http://www.robvanmeel.nl/?q=content/tables-regulations-army-books-and-forms-and-stationary-taken-field-infantry-battalion-9-p Perhaps fv1620 has the full list? Chris Henschke Posted 1 November , 2013 Share Posted 1 November , 2013 Army Form L 1398-23, was an about crown octavo sized pamphlet with about five pages of text, specific to an ASC unit in the field. The version that I found had an imprint date of 11/14.

Chris Henschke Posted 2 November , 2013 Share Posted 2 November , 2013 I have no idea of the title of Army Form O 1687, but have included an image of it. I have also included a fuller title for Army Form O 1800 so that the difference between it and Army Form W 3504 is more obvious. IPT Posted 2 November , 2013 Author Share Posted 2 November , 2013 Thanks again, Chris Henschke and David Porter for the latest additions. I've added them in. We've got a couple of O-1800 versions, but I think I've seen different wordings on this form in the service files so I may leave it at "Application for Issue of Railway Voucher". N 1510 is currently entered as both "Detachment Pay Sheet" and "Pay and Mess Form". They may even be the same thing? fv1620 - Thanks for the L's - Chris will be delighted to get his L 1398s. I hope it wasn't raining on the way to the wifi hotspot! We're up to 700 Army Forms now (plus another 70 from the scans above which I will add later). It's a bit daunting that we previously had 6 'L' forms, and we've just been given 80 of them. I would be interested (and scared) to know how many of the 200 pages are lists of Army Forms, at about 40 per page). Grumpy - Once we get as far as we can with this list, it would be good to denote (or even link to) those forms on the main list for which there is an example available? Chris Henschke Posted 2 November , 2013 Share Posted 2 November , 2013 fv1620, Thanks for attaching the scans - a great addition. Can you please tell me the name and date of the reference? Is it an Army Order? And at last...there really was an Army Form L 1375!! fv1620 Posted 2 November , 2013 Share Posted 2 November , 2013 Chris curiously there's no WO Registry File No. "Classified List and Alphabetical Index of Army Forms and Books (Including alterations notified in AO October 1916." The imprint shows December 1916 Front cover says Issued with AO dated 1st January 1917. If the weather improves I'll go up the road to the hot spot & post a picture of the cover. In the meantime has anyone got a reasonable resolution scan of L 1372 please? Clive

Chris Henschke Posted 7 November , 2013 Share Posted 7 November , 2013 I have just found a reference for three more forms in SS 393 Censorship Orders and Regulations for Troops in the Field, dated September 1917. 'Other field service post cards e.g. (Army Form A.2042A, 2042B and 2042C) provided they are used only for the purposes for which they are issued.' I remember seeing one in Maltese. IPT Posted 8 November , 2013 Author Share Posted 8 November , 2013 Field Service Post Cards A 2042A and A 2042B seem to have been variations of "Advice of Admission to Hospital". I haven't quite found the distinction yet. A 2042C seems to have been a re-direction postcard, advising the Army Postal Service of a change of address. With regard to the W's, i'll aim to get them updated in the next day or two. I'm still struggling to find a reference to the FSR Pt II 1930 forms in the Great War. They were clearly in use in WW2, but why bring in P.O.W. forms in the interwar years? A bit of a puzzle.
