Reason for discharge

Could someone please read the attached and tell me what it says exactly; I can see this soldier was admitted to the London General Hospital but my old eyes are not as good as they were. He was eventually discharged and I am trying to learn why he was disabled. Appreciate any help

Nephritis Acute I think it says. Specifically admitted to the Third London General Hospital in Wandsworth (in the Victoria Patriotic Building by Wandsworth Common).
