Can someone please help me with a transcription and interpretation of the attached detail from Arthur Allen Smith's Casualty Form - Active Service.

So far I think I've got:

"Adm 13 C.C.S. P.N.O. In the Fld 18/9/16
Trans to 27 Train "
To England per H.S 27/9/16
Cambria ex / Can. G.P."

I can't read the line inserted between "27 Train" and "To England"

I can't understand most of it other than he was transferred to England.

The previous pages show a history of misdemeanours resulting in various spells of Field Punishment No.1 between 12/11/14 and 2/6/16 including one of 3 months and culminating in one of 84 days which must have taken him pretty close to the first date above.

He was at Mebsey Park Military Hospital, Birkenhead. 28/9/16 - 14/10/16 with Rheumatism and transferred to the reserve on 3/7/19

I would say the form reads:

Admitted to No. 13 C.C.S. (Casualty clearing Station) P.U.O. - Pyrexia (fever) of unknown origin

Transferred to No. 27 (Hospital) Train (a railway train)

To England per (aboard) H.S. (Hospital ship) Cambria ex (previously at) No. 1 Canadian General Hospital
