I've been reading the war diary of the 24th Battalion, Victoria Rifles of Canada, and the earliest entries were written by an officer who had been appointed the brigade S.M.O.

Does this stand for Sanitation Medical Officer?

SMO stands for senior medical officer, I could be wrong but believe this to right

Oddly enough, a medical officer attached to a BATTALION was usually called a Regimental Medical Officer, who worked at the Regimental Aid Post. And, no doubt, sometimes treated the Regimental Sergeant Major of the Battalion. Daft, really.
